programming can kill?!

Our teacher shared to us that there was an incident that a programmer forgot to put limitations into his program for a X-ray machine... because of this many people died...

i think its over exposure due to radiation...
And its not only the programmers fault that goes into the picture.. even products like X-ray machines undergo quality control to prevent events like that from happening. So more or less, the whole company that manufactured the x-ray machine was sued rather than the programmer himself/herself.. The programmer did what he was asked to, yes he made a miscalculation and that's why there's a dept. called quality control to ensure to every end product is in tip-top shape and to ensure there are no defects before distribution...
